  • Unlimited access to Care Management (M-F : 8AM-8PM; S-S : 10AM-2PM)
  • Rapid access to Mental Health assessments and support
  • Proactive care from our Health1Pro team of Medical Professionals

On an average a callback from the Doctor is within 60 minutes or less. Every member will be assigned a personal Care Manager in our Care Management, Navigation, Patient Advocacy department.

The $50 per month covers the Primary Member and 3 dependents. For an additional $10 per month, it will cover the Primary Member and five dependents for a total of six people on your Plan.

Membership Cost Description:

There is a one-time set-up fee of $45. The complete, transparent cost to be a member is:
 

Basic Plan: $95 one-time (includes first month) then it is $50 per month for a Plan of Four.
Or, $105 one-time (includes first month) and then it is $60 per month for a Plan of Six.
 

Premium Plan: $130 one-time (includes first month) then it is $85 per month for a Plan of Four.
Or, $140 one-time (includes first month) and then it is $95 per month for a Plan of Six.
Health1Pro Premium Plan provides all the services in the Basic Plan plus the assignment of a Virtual Primary Care Physician. This provides an appointment with the same Doctor with 24-hour notice.
