Heath1Pro Annual Healthcare and Wellness Agenda for Individuals and Families

The Health1Pro Six Step Care Management System:

Important note: You have same day access to Telehealth appointments. Depending on your program it could be with Urgent Care or your Primary Care Physician within 24 hours. However, if the need arises before the Orientation call with your Personal Care Manger, you’ll need to call us to set the appointment.

Step 1: Meeting your Personal Care Manger.

Within three days of completing sign up, you will have your first call with your permanent Personal Care Manager (PCM). This call will include:

  • 1.) A complete orientation on how to maximize the benefits of your H1P membership.
  • 2.) Completing the comprehensive Personal Health History (PHH) for you and each of your dependents on your H1P account.
  • 3.) Take information for our legal team to prepare your three highly important legal documents
    • a.) Living Will,
    • b.) Medical Power of Attorney,
    • c.) Personalized Do Not Resuscitate. Step 1 is completed quickly, and the legal documents are ready and delivered within 30 days.

Step 2: Building your Personal Health History.

Following your orientation call our team goes to work resourcing and compiling all your known medical records and reports, organizing them into one usable comprehensive records management system. This process takes an average 60 days to complete as it requires our team to reach out to various healthcare providers.

Step 3: Developing your Annual Personal Healthcare and Wellness Agenda.

Upon completion of Steps 1 & 2 the Annual Personal Healthcare & Wellness Agenda for you and your dependents can be completed. This will include next steps and timelines based on the health status and needs of each person on your H1P account. Our goal is to finish these first three steps within 90 days, so that your Annual Healthcare & Wellness Agenda is complete.

Step 4: Your Plan is in full process.

Using your completed Annual Personal Healthcare & Wellness Agenda, any action steps will be implemented to support your needs, whether you are in good health or are experiencing health concerns. In either instance, our care management team is proactively working to support you in maintaining your wellness or regain your health.

Step 5: Estate Plan

In the fourth quarter of Year 1 as a member of Health1Pro your PCM will suggest you start the process of putting an Estate Plan in place. If you are ready, your PCM will make an appointment for a confidential meeting with one of our legal staff to begin assessing what your Estate Plan needs are. BTW, this service is included in your H1P membership.

Step 6: Managing and Maintaining the Health & Wellness of you and your family in an ongoing basis.

IOne of our moto’s at H1P is “an ounce of prevention is worth a pound of cure.” Our system is designed to ensure you have the support you need in maintaining your wellbeing. And we also know that it requires ongoing effort to stay current and maximize health and wellness. Our Care Management System is designed to take the burden off you in keeping up with your health and wellness. H1P is a proactive system of ongoing support from Telehealth Urgent Care needs to a well thought out plan for ongoing Wellness.
