Terms of Service

1. Acceptance of Terms

By enrolling in or using Health1Pro's services ("Services"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, you may not use the Services.

These Terms constitute a legally binding agreement between you ("Member," "you") and Health1Pro LLC ("Health1Pro," "we," "us").

2. Nature of Services

Health1Pro provides a membership-based healthcare support platform, which may include:

• Virtual urgent care services
• Virtual primary care services
• Care management and coordination
• Patient advocacy (including, where available, in-person support)
• Behavioral and mental health support
• Assistance with medical records organization
• Health-related legal documents (e.g., medical power of attorney, living will, DNR)
• Access to third-party services such as laboratory testing (when applicable)

Health1Pro is not an insurance company. Membership fees do not cover all healthcare services or expenses.

3. No Insurance / No Guarantee of Coverage

Health1Pro is not health insurance and does not replace insurance coverage.

• You are responsible for any healthcare services received outside of Health1Pro.
• Health1Pro does not guarantee coverage, reimbursement, or payment for any third-party services.
• Membership provides access to certain services, not unlimited care.

4. Medical Disclaimer

• Health1Pro facilitates access to licensed healthcare providers but does not itself provide medical care.
• Providers you interact with may be independent contractors or third-party providers.
• All medical decisions are made between you and your provider.

Emergency Situations: If you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room. Health1Pro is not an emergency service.

5. Eligibility

You must be at least 18 years old to enroll. A parent or legal guardian may enroll dependents under a family plan.

You agree to provide accurate and complete information and to keep it updated.

6. Membership Plans and Fees

Health1Pro offers subscription-based membership plans, which may include:

• Urgent Care Plan
• Individual Primary Care Plan
• Family Primary Care Plan

Billing Terms:

• Membership fees are billed monthly unless otherwise stated.
• Fees are non-refundable except as required by law or explicitly stated.
• Pricing may change with notice.

7. Cancellations and Termination

• You may cancel your membership at any time.
• Cancellation will take effect at the end of the current billing cycle unless otherwise specified.
• Health1Pro reserves the right to suspend or terminate accounts for misuse, fraud, or violation of these Terms.

8. Use of Services

You agree not to:

• Misuse or abuse the Services
• Provide false or misleading health information
• Attempt to access services fraudulently
• Interfere with system operations or security

Health1Pro reserves the right to limit or deny access for inappropriate use.

9. Third-Party Services

Health1Pro may connect you with third-party providers, including laboratories, specialists, and social services organizations. We are not responsible for the performance, accuracy, or outcomes of third-party services.

10. Privacy

Your use of the Services is also governed by our Privacy Policy. Health1Pro takes reasonable measures to protect your personal and health information in accordance with applicable laws, including HIPAA where applicable.

11. Intellectual Property

All content, branding, and materials provided by Health1Pro are the property of Health1Pro LLC and may not be used without permission.

12. Limitation of Liability

To the fullest extent permitted by law:

• Health1Pro shall not be liable for indirect, incidental, or consequential damages.
• Total liability shall not exceed the amount paid by you to Health1Pro in the preceding 12 months.

13. No Guarantees

Health1Pro does not guarantee:

• Specific health outcomes
• Availability of providers at all times
• That all services will be suitable for your needs

14. Geographic Limitations

Services may vary by state and are subject to provider licensing laws. Health1Pro does not guarantee availability of all services in all locations.

15. Modifications to Terms

We may update these Terms from time to time. Continued use of Services after updates constitutes acceptance of the revised Terms.

16. Governing Law

These Terms shall be governed by the laws of the State of Arizona, without regard to conflict of law principles.
